SPRAVATO® and major depression with suicidal thoughts or actions

SPRAVATO® is FDA-approved, together with an oral antidepressant, for adults with major depressive disorder who have acute suicidal ideation or behavior — as part of urgent, supervised care.

The short version

  • Approved with an oral antidepressant for adults with MDD and acute suicidal ideation or behavior
  • Reduces depressive symptoms; not proven to prevent suicide
  • Not a substitute for hospitalization when hospitalization is needed
  • In crisis: call or text 988, or call 911

The approved use

SPRAVATO®, taken together with an oral antidepressant, is FDA-approved for adults with major depressive disorder (MDD) who are experiencing acute suicidal ideation or behavior. In studies, it reduced depressive symptoms rapidly; it has not been shown to prevent suicide or reduce suicidal behavior itself, and it does not replace hospitalization when that is needed.

If you are in crisis right now

Call or text 988 (Suicide & Crisis Lifeline) or call 911. SPRAVATO® treatment is planned care, not an emergency response.

What treatment involves

Dosing happens only at a certified treatment center, with at least two hours of monitoring after each dose. SPRAVATO® carries boxed warnings for sedation, dissociation, respiratory depression, abuse and misuse, and suicidal thoughts and behaviors, and it is not approved for children or adolescents.

What a course of treatment looks like

  1. 1

    Consultation and eligibility

A qualified healthcare professional reviews your diagnosis, treatment history, medicines, and safety risks before determining whether SPRAVATO® is appropriate.

  2. 2

    Insurance authorization

If required, the clinic submits clinical documentation to the insurer. Requirements and timing vary by plan and are not guaranteed.

  3. 3

    A monitored treatment visit

You self-administer the prescribed nasal spray under direct observation in a certified healthcare setting.

  4. 4

    Observation and transportation

The clinic monitors you for at least two hours after each dose. Arrange a ride home and do not drive until the next day after restful sleep.

  5. 5

    Individualized follow-up

The prescriber assesses benefit, tolerability, and ongoing need. Dose and frequency depend on the labeled indication and individual response.
